Débuter avec une faible dose. Surveiller étroitement les signes et symptômes de toxicité et ajuster la dose selon l’efficacité et la tolérance.

Symptômes de toxicité associés aux hypolipidémiants : effets gastro-intestinaux, fatigue et faiblesses musculaires, myalgies, crampes musculaires, myopathies, rhabdomyolyse et myoglobinurie conduisant à l'insuffisance rénale.

Commentaire

Association généralement bien tolérée, il est cependant préférable de commencer la pravastatine à faible dose car il y a une grande variabilité interindividuelle.

Réf #2380 : La co-administration la pravastatine avec le darunavir/ritonavir a augmenté la SSC de la pravastatine de 21%.

Réf #2129 : Cependant, dans une autre étude avec darunavir/ritonavir, on a observé une augmentation moyenne de la SSC de la pravastatine de 81% avec des augmentations allant jusqu'à 5 fois chez quelques sujets.

Réf #3391 : Rapport de cas de rhabdomyolyse chez un patient sous élvitegravir/cobicistat et pravastatine/fénofibrate.
