Réf #3447 : Une étude croisée randomisée a évalué l'impact des antirétroviraux potentialisés sur la pharmacocinétique des métabolites actifs du clopidogrel et du prasugrel et sur leur efficacité chez des patients séropositifs. Chez les patients recevant le clopidogrel 300 mg en concomitance avec un traitement contenant ritonavir ou cobicistat, on observe une diminution significative de 69% de la SSC et de la Cmax du métabolite actif du clopidogrel comparativement aux patients en santé recevant seulement le clopidogrel. L'inhibition plaquettaire était insuffisante chez 44% des patients sous ritonavir ou cobicistat alors qu'elle était adéquate chez tous les patients sans ritonavir ou cobicistat. Pour les patients recevant le prasugrel 60 mg avec le ritonavir ou le cobicistat, on observe une diminution de 52% de la SSC et de 41% de la Cmax du métabolite actif du prasugrel. Par contre, tous les patients sous prasugrel montrent une inhibition plaquettaire puissante. Les auteurs concluent que le prasugrel reste un agent antiplaquettaire adéquat chez les patients infectés par le VIH et pourrait être préféré au clopidogrel dans ce contexte, indépendamment de l'interaction métabolique et de l'inhibition de ses voies de bioactivation.

Réf #3449 : Une autre étude chez des patients en santé recevant clopidogrel (300 mg au premier jour suivi de 75 mg QD( et ritonavir (100 mg BID) a montré une diminution de la SSC de 52% et de la Cmax de 49% du métabolite actif du clopidogrel. L'inhibition plaquettaire moyenne et maximale ont été réduite à 31% et à 40%, respectivement. Les auteurs concluent qu'il est recommandé d'éviter l'administration concomitante de clopidogrel avec des traitements contenant du ritonavir.

Réf #3448 : Rapport de cas d'un patient de 45 ans séropositif et traité avec darunavir/ritonavir et emtricitabine/ténofovir. Suite à une intervention coronarienne, le patient débute un double traitement antiplaquettaire avec de l'aspirine et du clopidogrel alors que son traitement antirétroviral était maintenu. Six mois plus tard, le patient présente un épisode de douleur à la poitrine avec des signes électrocardiographiques d'infarctus du myocarde antérieur aigu et des signes de thrombose du stent implanté. Suite à une nouvelle intervention, le clopidogrel est remplacé par le prasugrel, tandis que le darunavir/ritonavir est maintenu. Aucun nouvel épisode de thrombose n’a été observé.
