Réf #122 : Une diminution significative de 86% de la SSC du cortisol plasmatique a été observée.

La pharmacocinétique du fluticasone (métabolisme par le cytochrome 3A4, longue demi-vie, biodisponibilité, volume de distribution, lipophilicité, etc) favorise son accumulation en présence d’inhibiteurs du cytochrome 3A4.

L’accumulation des corticostéroïdes peut engendrer un syndrome de Cushing (faciès lunaire, excitation/insomnie, hypertension, augmentation de l’appétit et du poids, facilité à faire des ecchymoses) et potentiellement une suppression de l’axe hypothalamo-hypophysaire qui pourrait mener à une insuffisance surrénalienne. Plusieurs cas ont été rapportés dans la littérature, et ce, dans un délai aussi court que 2 semaines.

Cette interaction a notamment fait l’objet d’une alerte Santé Canada en 2004 et la monographie du ritonavir précisent que son association avec le fluticasone constitue un risque d’effets secondaires sérieux.

Le corticostéroïde le plus sécuritaire est le béclométhasone (Qvar) étant donné que son métabolisme se fait surtout par un mécanisme d’estérification.

Réf #2550 et #2551 : Une étude transversale (n = 11 783) suggère la prévalence moins élevée d’insuffisance surrénalienne avec le béclométhasone qu’avec les autres corticostéroïdes actuellement disponibles sur le marché. Des études pharmacocinétiques avec le béclométhasone et le ritonavir ou la combinaison darunavir/ritonavir ont démontré qu’il n’y avait pas d’interaction pharmacocinétique cliniquement significative. Aucune variation du cortisol sanguin n’a été observée.

Le fluticasone ne devrait pas être arrêté sans consultation avec un médecin. Si l’axe hypothalamo-hypophysaire est supprimé, l'arrêt soudain du fluticasone peut mener à des signes et symptômes d’insuffisance surrénalienne. Il est recommandé de diminuer lentement le corticostéroïde pour éviter les symptômes de sevrage (fatigue, perte de poids, étourdissements, faiblesse, hypotension posturale et crise surrénalienne aiguë). Un test de cortisol plasmatique et de l’ACTH permettra de documenter la présence ou non d’insuffisance surrénalienne.
